ministry volunteer liability release

I hereby acknowledge that it is my desire to participate/allow my child(ren) to participate in church sponsored ministries and activities at Impact Church including activities on and/or away from the church premises.

 

I am/My child(ren) is voluntarily participating in these ministries and activities, including transportation to and from such activities, if applicable, and accept any assigned duties with knowledge of the dangers involved and hereby agree to accept all risks of injury as a result of such participation.

 

I understand that I/my child(ren) may appear in printed and digital media footage used by Impact Church.

 

As lawful consideration for participating in such ministries and activities, I do hereby agree to indemnify and hold harmless Impact Church, its officers, employees, volunteers and members of the Board of Directors from any and all claims, lawsuits, demands, causes of action, liability, loss, damage and/or injury, of any kind whatsoever (including without limitation all claims for monetary loss, property damage, equitable relief, personal injury and/or wrongful death) that I and my heirs, distributes, guardians, legal representatives or assigns, now have or may hereafter have arising out of, in any way whatsoever, any acts, omissions or negligence before, during or after my participation in such church sponsored ministries and activities on and/or away from church premises.

 

I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability and assumption of risk agreement, and I sign it as an intentional act.

 

This release of liability and assumption of risk agreement shall remain effective until revoked in writing and delivered to an employee, agent or officer of Impact Church, appointed to receive such documentation.

Ministry Volunteer Media and Creative Rights Policy

In exchange for serving as a ministry worker/volunteer or participant in a Impact Church service or event, I grant to Impact Church, it’s legal representatives and assigns, the right and permission to use my and/or my child’s name and likeness and to use and publish photographic pictures, video, footage and sound recordings of me (the “Materials”) and/or my child(ren) during church-related events and activities, whether via print, digital, broadcast or in any other media and for any lawful purpose. I release, discharge and agree to hold Impact Church harmless from any liability for any use of the materials.

 

Further, if I or my child(ren) serve as a volunteer photographer/videographer/writer for Impact Church (the “Church”) for any of its activities or events (the “Activity”), in exchange for my/my child(ren) participation, I agree to assign all ownership in the photos/videos/content/creative materials created by me/my child(ren) during the Activity (the “Materials”) to Impact Church for broadcast, publication or any lawful purpose through any media, at the Church’s sole discretion. I acknowledge that such Materials will be considered a “Work Made for Hire” as defined under U.S. Copyright Law.

 

I further agree that I or my child(ren) will not display, use, distribute or sell the Materials without permission from the Church. I understand that by providing my contact information listed on the applicable Impact Church Form, I agree to be contacted by Impact Church via mail, telephone, email or text message.
